Got Transition, National Health Care Transition Center:  Got Transition? is a national resource for health care professionals, families, youth, and state policy makers focusing on a young adult's transition from pediatric to adult health care. Starbright World: Starbright World is a virtual hangout where you can build on existing friendships or create new ones, from home or from the hospital. Starbright World is an online social network where teens (ages 13 to 20) who have serious medical conditions, and siblings of seriously ill teens, can connect with each other via moderated chat rooms, games, bulletin boards, videos, and more:

JobTIPS is a free, Web-based program that helps youth with ASD or other disabilities develop and maintain skills needed for successful employment. The site offers resources for educators, job coaches, clinicians and transition coordinators who support individuals with ASD as they transition out of high school and into vocational education or other post-secondary activities.

Kids As Self Advocates (KASA) is a national, grassroots project created by youth with disabilities for youth and is part of the Family Voices Network.

 

We are teens and young adults with disabilities speaking out. KASA knows youth can make choices and advocate for themselves if they have the information and support they need.

 

We are leaders in our communities, and we help spread helpful, positive information among our peers to increase knowledge around various issues. We also help health care professionals, policymakers and other adults in our communities understand what it is like to live our lives and we participate in discussions about how to help each other succeed.
