Tools for Health Care Providers, Families, Youth and Young Adults

Health care transition is the process of changing from a pediatric to an adult model of health care. The goal of transition is to optimize health and assist youth in reaching their full potential.

 

To achieve this goal requires an organized transition process to support youth in acquiring independent health care skills, preparing for an adult model of care, and transferring to new providers without disruption in care.

 

This web-site includes a number of tools and resources to assist providers, families and youth in making the transition from pediatric to adult health care.  

 

These tools and resources include tools that have been tested around the country and by clinics and families in Minnesota through the Minnesota Transition Collaborative facilitated by Family Voices of Minnesota and funded by the Children and Youth with Special Health Needs Section at the Minnesota Department of Health and the National Health Care Transition Center.
